We find that explaining everything first goes a long way towards relieving your fears. Doesn’t that just make sense?

To save you time in our office, we ask that you print off the New Patient Intake Forms from this website, fill them out at home and bring them with you.

  • First Visit. To To find out if we can help you we’ll need to gather some information first.
     
  • Second Visit. We’ll report what we found and answer all your questions.
     
  • Regular Visit. A typical visit will take less time, but is just as important.
